1. Required documents for applying for compensation are: Application Form for Student Group Insurance Compensation, diagnostic certificate, receipt, copy of applicant’s account passbook, X-ray film (when there is fracture).
    
2. Application must be made within two years after the accident occurred.
    
3. Compensation will be issued for hospitalization or accidents.
