1. No! A supporting document from a private clinic or hospital is also acceptable.
2. However, the supporting document should clearly state reason that the student saw the doctor (e.g., a serious cold, …), why the student could not take the exam. (must stay home and rest for one day, admitted to the hospital/clinic for treatment, …), the date the doctor saw the student (proving that the student could not come take an exam on a specific day). Of course, the document must bear the official square seal mark hospital and the doctor's seal or signature!

The carriers need not to be isolated; therefore, it should be treated calmly.
